National Action Plan on Prevention and Containment of AMR (2017-2022)
This is the first NAP on the Prevention and Containment of AMR in Kenya. It has been developed based on the National Policy on Prevention and Containment of AMR and the recommendations of the situation analysis on AMR conducted in 2011 and updated in 2016. This strategy provides a regulatory and implementation framework to establish and strengthen systems to contain the emergence and spread of AMR. Implementation of this strategy will require substantial funding and high-level political commitment. Because AMR is a multidisciplinary and intersectoral issue, successful implementation of this strategy will require effective coordination and collaboration among different sectors.

This guideline provides an avenue to comprehensively evaluate the wide range of interventions that can be implemented by AMS programmes in different health care settings in light of different resource capacities. It also addresses approaches to measure the success of these interventions and the structure of an AMS programme, emphasising the importance of physician and pharmacist leadership, robust microbiology capacities, infectious disease expertise, and measurement and feedback as critical components of AMS programmes. This guideline will be backed by strong government and institutional commitment and collaborative actions across the disciplines and calls on everyone to act now to avert the threat of AMR in Kenya and the world.

The Ministries responsible for Health and Agriculture appointed a National Antimicrobial Stewardship Advisory Committee (NASAC) in 2014, which has provided the necessary multi-sectoral coordination for the AMR agenda, including the development of the AMR Policy and National Action Plan. Through the NASAC, a broad stakeholder base was consulted, involving national departments and agencies, county departments of health, agriculture, veterinary, fisheries and the environment; as well as non-state implementing partners and development partners. These stakeholders have contributed data, information, experiences and best practice; thereby shaping a comprehensive nation-wide agenda to combat the threat of AMR.

All stakeholders are advised to align their internal M&E systems to this M&E framework and commit to reporting on it.

The Kenya Essential Medicines List (KEML) is a key tool that should effectively be used to promote access to essential medicines, and through their correct selection, procurement, and use to achieve maximum therapeutic benefit and optimize patient outcomes as desired under Universal Health Coverage (UHC). The rationale for listing medicines in the KEML 2023 was adopted from the globally coordinated process by the World Health Organization (WHO), which develops the Model Lists of Essential Medicines for both adults (23rd List, 2023) and children (9th List, 2023), and makes relevant information and knowledge available to countries for their own adaptation. The National Medicines and Therapeutics Committee (NMTC), through a Technical Working Group (TWG), coordinated collection and adaptation of clinical evidence, held extensive stakeholder consultations, and referenced updated clinical and disease specific guidelines for this updated list.

The Kenya Government is implementing universal health coverage (UHC) as one of the priorities of the Kenya Kwanza manifesto. UHC aims to ensure that all citizens have access to safe, effective, quality essential health care services, which includes affordable essential health products and technologies (HPT) without suffering financial hardship. UHC is anchored in the Kenya Health Policy 2014-2030 and includes primary health care. The focus of UHC is not only on preventing and treating disease and illness but also on improving the population’s general wellbeing and quality of life. The Health Products & Technologies Supply Chain Strategy 2020-2025 identified the lack of a National Formulary in Kenya as a major gap that needs to be addressed. The National Medicines and Therapeutics Committee (NMTC), which was inducted in September 2019, has 12 terms of reference, one of which is the development of a National Formulary.

The National Action Plan (NAP) for the prevention and containment of Antimicrobial Resistance (AMR) in Kenya, provides an implementation framework to support the containment of AMR. These efforts are led by the ministries health, agriculture, environment, and fisheries appointed in the National Antimicrobial Stewardship Interagency Committee (NASIC). The Monitoring and Evaluation (M&E) Framework, aligned to the NAP was developed, and will be used to track the implementation and assess the outcomes of implementing the NAP-AMR 2023-2027. It also outlines the key indicators and the roles and responsibilities for the different partners and stakeholders in the M&E process. The framework is to be implemented in conjunction with the annual operational plans and the Communication Strategy for the Prevention and Containment of AMR.

Antimicrobial resistance (AMR) occurs when bacteria, virus, fungi, and parasites change over time and no longer respond to medicines making infections difficult to treat. This increases the risk of disease-spread which may lead to severe illness and death. The currently available standard treatments to common infections have become ineffective, with a rapid rise in multidrug resistant organisms (superbugs) globally, resulting to increased vulnerability of the community to infections. The Ministries of Health (MOH); Agriculture and Livestock Development (MOALD); Mining, Blue Economy and Maritime Affairs (MIBEMA); and Environment, Climate Change and Forestry are devoted to demonstrating stewardship in the implementation of this National Action Plan (NAP) on prevention and containment of AMR. This NAP is in line with the National Policy on prevention and containment of AMR and is a product of the NAP-AMR (2017-2022).

The prevention and control of infections are essential cornerstones of clinical care in all health care settings. The changing pattern of infections and the emergence of bacteria with multiple-antibiotic resistance emphasize the need for all health care workers (HCWs) to understand and practice evidence-based infection prevention and control (IPC) practices that will protect patients, clients, and health care workers from health care-associated infections (HAIs). Health care-associated infections include urinary tract infections, surgical-site infections, bloodstream infections, and pneumonia. They are a major problem for patient safety—leading to prolonged hospital stays, long-term disabilities, increased resistance of microorganisms to antimicrobials, massive additional financial burdens, high costs for patients and their families, and deaths. As such, the prevention and control of HAIs must be a top priority for institutions that are committed to making health care safer.

The Constitution of Kenya and the Kenya Health Policy, 2014–2030 demonstrates the health sector’s commitment to ensuring that the country attains the highest possible standards of health, in a manner responsive to the needs of the population.
